Description:
Shared Lives Officer To support the effective delivery of Enfield’s Shared Lives scheme, a CQC-registered adult social care service that enables adults with eligible care and support needs to live safely, independently and as part of a Shared Lives household and local community. The post holder will provide person-centred support to Shared Lives carers and adults in placement, helping to ensure arrangements are safe, well-matched, stable, regularly reviewed and responsive to assessed needs, outcomes, wellbeing, dignity and choice. The role will support key scheme activity including carer recruitment, assessment, training, approval, matching, monitoring and review, while promoting the Shared Lives model of care and support within an ordinary family and community setting. The post holder will work in line with CQC requirements, safeguarding duties, Council policies and Shared Lives good practice, maintaining accurate records, identifying risks or changes in need, and working with carers, adults, families, social care colleagues and partner agencies to support safe and sustainable placements.
